Form A is required by the Diocese of Fort Worth and is mandatory for each event. The great news is that you only have to fill it out once each year. Form A is good for the whole year. You will not need a notary to sign unless you think your child/children will join us for an event outside of Texas. Form B is also required for each event and Form B will give the specifics of each event. Form B (Permission Slips) can be downloaded on Sainteasy.org and can be found attached to the event (look on the calendar to find the event and download the Form B)
